Our medical history forms are located below. Please fill out completely to the best of your ability, but not all sections may apply to you or your child. If there are any questions, please do not hesitate to reach out.

If you need records sent to us, please print a Records Release Form. Fill it out completely and return to us so we can contact your previous or referring doctor.

Zenith Vision Development Center is not contracted with any insurance plans and all payments for services and evaluations are due at the time of the appointment. A list of our common services and professional fees are included below.
