Insurance and Payment Information

Zenith Vision Development Center does not accept assignment from insurance companies and is considered a private pay or out-of-network provider. All fees are the responsibility of the patient/parent/guardian. We require payment in full at the time of service.

Our office accepts cash, checks, and credit cards. Patients may also utilize Care Credit, Health Savings Accounts and Flexible Spending Accounts. An itemized invoice will be provided after each visit that can be submitted to your insurance if needed.
